Having attended an initial screening with the Disability Service it has been recommended that I attend Educational Psychologist Assessment (EPA).

If I choose to attend the EPA, I will pay a personal contribution of £100 towards the cost. I understand that the full assessment costs the university £265.00 and that the balance of this will be met by the University.

I understand that if I do not attend my appointment or give less than 24 hours’ notice to rearrange, the £100 personal contribution I have paid for this assessment will be treated as a non-refundable deposit payable in advance of my appointment.

I agree to Manchester Met University passing my name and contact details to EGS for the purpose of booking an appointment
